My duties include:-
- Taking histories from patient, examining and sending relevant tests when necessary.
- Managing acute emergencies which include ACS, Stroke, Acute exacerbations Of Asthma/COPD, DKA, poisoning, polytrauma etc.
- Our hospital in located near the highway so Trauma is one of the most common cases. Managements such as application of back slab, closed reductions with Xray and POP are done in the department.
- Admission of patients and initial management plan after discussion with the relevant consultant. After that taking to the doctor looking after the ward and handing over the patient.
- Making swift and accurate decisions regarding the further action needed.


Communication Role:-

- Emergency department is happening department where multiple patient come with severe concerns regarding their health. It is my duty to not only manage the patient but to explain and address their concerns with empathy and clear words.

- The counseling also extends to family member who are concerned regarding their patient's health.

- During my 4 weeks. I did 4 rotations which included Acute assessment unit, Respiratory Unit, Gastroenterology Unit and lastly, Elderly Care Unit. My duties included:-

-Shadowing healthcare professionals to observe and learn clinical procedures.
-Gaining insights into patient interactions and communication such as palliative care counseling.
-Learning and following protocols for hygiene and infection control.
-Attending ward rounds with multiple consultants to understand both patient cases and treatment plans and also to understand how every consultant approaches different cases.
-Contributing to discussions when appropriate under supervision and timely attending MDT meetings.
-Learning the proper use of electronic health record system followed in Bedford Hospital including how noting, requests, prescriptions, discharges and e-referrals
are made.
-Observed and learned the process of cannulation, ultrasound guided cannulation, blood sampling, ultrasound guided ascitic tap and how to send off samples to pathology.
-Understanding the roles of different healthcare professionals.
-Respecting diversity and cultural considerations in patient care.
-Observing colleagues during important discussions with patients and relatives including discussions about ceiling of care and breaking bad news.

Communication Role:-

- I have done my Plab Examinations. There is a separate portion of marking for IPS (Inter-personal skills) and I was able to apply these during my rotation.

- I observed how the healthcare system worked in the UK and understood how everything is patient centered as it should be.

- I learned how to deal with angry patients, Breaking bad news, Medical errors and multiple other communication challenges and I am confident that I can handle any situation.

Worked as a Medical officer in RIMS Trauma Hospital which is tertiary care hospital. My role in the hospital included :


  • Looking after patients in both Medical and Surgical wards. The Surgery Department included Orthopedics, ENT, Urology, Neurology, Maxillofacial and General Surgery departments.
  • I was assigned to take history from the patient, general assessment and general examination, managing their active complaints and symptoms, discussing medications with consultants and providing the patient with the best care possible.
  • Counseled both surgical and medical patients about their condition and outcome of the procedures. Counseled them in detail about the procedure, pre and post op care, managed their active complaint, wound care and discussing all the management with the consultants.
  • I have also worked in the emergency department and ICU of Rims trauma hospital and taken care of critically ill patients.

I completed 4 rotations:-


ORTHOPEDICS AND TRAUMA

My role was divided in 3 groups:-

  • WARD:-
  • Managed and reviewed multiple patients with different cases.
  • Counselled patient’s regarding pre and post op care. Informed in detail about the procedure and complication of the required operation.
  • Role also included taking consent and taking post op precautions, which included wound care, bandaging and looking out for signs of infections

2. OPERATION THEATER:-

  • My duty required me to prepare the patient prior to surgery which included draping.
  • Scrubbing and assisting in multiple operation such as Total Knee Replacement, Total Hip Replacement, Promixal Femur replacement and many more. Furthermore, have closed and sutured multiple operative wounds.
  • Writing the patient’s post op notes under consultant supervision.

3. OUTPATIENT CLINIC:-

  • Attached myself to the consultant during his opd hours and learned to intrepret X-rays and CT-Scans and multiple other imaging.


GENERAL SURGERY

My role in this rotation was divided into 4 groups.

1. SURGERY WARD:

  • Reviewed in-patient beds allocated to me, including monitoring their vitals, IO charting, drain charting, blood sampling, antiseptic wound dressing and requesting/chasing investigations.
  • Presented cases in the morning rounds with consultants and followed up on their orders.
  • Maintained patient files, arranged referrals, discharge summaries and counseled patients.
  • Counseled patients regarding medicine intake and wound care at home. .
  • Prepared patients for procedures, this involved pre-operative assessment, counseling and assessments by anesthetists.
  • Attended daily morning presentations and meetings.

2. OPERATION THEATER:

  • I observed and assisted some major and minor surgeries such as cholecystectomy (open and laparoscopic), hernia repair, foreign body removal, Hemorrhoids, MRM, emergency laparotomy, appendectomy, thyroidectomy, incision and drainage, cyst removal and breast biopsies.
  • I learnt stitching and suturing along with other surgical techniques used in open and laparoscopic surgeries.

3. SURGICAL OPD:

  • Attended out-patient general surgery, colorectal carcinoma, thyroid and breast clinics supervised by consultants.

4. EMERGENCY ROUNDS:

  • Managed patients with surgical emergencies in teams such as cholecystitis, appendicitis, strangulated hernia, traumatic abdominal injuries, stab wounds, abdominal obstructions, abscesses, testicular torsion and acute pancreatitis.


GENERAL MEDICINE

My role was divided in to 3 groups:

  • WARD:-
  • Reviewed in-patient beds allocated to me including monitoring patient vitals & RBS, performed IO charting and requested appropriate routine investigations to assess patient progress.
  • Maintained patient files, arranged referrals, discharge summaries and counseled patients.
  • Regular ward rounds with consultants and followed up on their orders.

2. EMERGENCY:-

  • Admitted patients (under consultant supervision) from the ER with acute medical emergencies. Example IC bleeds, upper GI bleeds, sepsis, DKA, metabolic acidosis, hepatic encephalopathy and hypoglycemia.

3. OUT-PATIENT CLINICS:-

  • Attended general medicine, endocrinology, rheumatology and nephrology outpatient clinics supervised by seniors and consultants.


NEPHROLOGY

My role was divided into 3 groups:-

  • DIALYSIS UNIT:-
  • Looked after and managed patients undergoing dialysis. Learned to operated the dialysis machine

2. WARD AND ICU:-

  • Looked after and managed under consultany supervision multiple case related to nephrology such as electrolyte imbalance, CKD, Kidney failure and many more.

3. OUTPATIENT CLINIC:-

  • Did regular OPD with consultant and managing emergency cases in emergency and admitting patients when required.
